I understand and agree that in participating in any dance class, training or workshop, there is a possibility of physical injury. I voluntarily agree to assume all risks and responsibility for any injury or accident, which might occur to my child during any class, rehearsal, performance, or activity. I agree to not hold iMpact Dance Intensive, any dance teacher, instructor, choreographer, trainer, or facilities liable for any injury including, but not limited to personal injury, illness, disability, loss, or death that may occur before, during, or after classes, workshops, performances, and rehearsals. I give my consent to iMpact Dance Intensive to use any media or medium of me, my child and/or family for posting on iMpact Dance Intensive social media pages as well as for advertising and/or promotional activities.
